Causes

The specified Fibre Channel host bus adapter might not be in a useable state. Depending on the configuration and the ability to fail over to another adapter, some LUNs might be inaccessible.

Resolutions

Check the cables, switches, and Fibre Channel host bus adapters. Make sure that all connections are clean and secure, and that no components are faulty.
