The following problem causes this event
SAS wide port lost link on PHY.
1) Unplug affected disk, wait 30 seconds, reinsert it. 2) Check/replace cable between controller and affected disk. 3) Check version of RAID controller/iRMC firmware, and BIOS. 4) Replace affected disk.
