Summary

The SCSI physical drive has failed.

Causes

This could be caused by the following:

Resolutions

Check the physical drive status in the event description. If failed(3) or predictiveFailure(8), the drive might need to be replaced. If notConfigured(4), you need to ensure that all of the drive's switches are properly set and that the configuration utility has been run. If badCable(5), check the cables connected to it.
